Rheumatoid arthritis (RA) is a systemic autoimmune disease characterized by inflammatory arthritis and extra-articular involvement. It is a chronic inflammatory disorder caused in many cases by the interaction between genes and environmental factors, including tobacco, that primarily involves synovial joints.[1] It typically starts in small peripheral joints, is usually symmetric, and progresses to involve proximal joints if left untreated.[2][3][4] Joint inflammation over time leads to the destruction of the joint with loss of cartilage and bone erosions. RA with a symptom duration of fewer than six months is defined as early RA, and when the symptoms have been present for more than six months, it is defined as established RA.[3] RA, if untreated, is a progressive disease with morbidity and increased mortality.[5]

Rheumatoid arthritis (RA) and ankylosing spondylitis (AS) are among the most common rheumatic diseases. These chronic progressive inflammatory diseases lead to a reduction in physical fitness and increase in joint degeneration.[1] Although very closely related, their symptomatology and etiology are different. In the past, AS was often diagnosed inaccurately as RA, but the two have recently been recognized as separate, distinct clinical entities. The latter has been attributed to advancing diagnostic and laboratory techniques able to differentiate the two conditions.[2] That being said, the diagnostic differentiation still remains a challenging task for clinicians.

Juvenile idiopathic arthritis (JIA) is the most common paediatric rheumatological disorder and is classified by subtype according to International League of Associations for Rheumatology criteria. Depending on the number of joints affected, presence of extra-articular manifestations, systemic symptoms, serology and genetic factors, JIA is divided into oligoarticular, polyarticular, systemic, psoriatic, enthesitis-related and undifferentiated arthritis. This review provides an overview of advances in understanding of JIA pathogenesis focusing on aetiology, histopathology, immunological changes associated with disease activity, and best treatment options. Greater understanding of JIA as a collective of complex inflammatory diseases is discussed within the context of therapeutic interventions, including traditional non-biologic and up-to-date biologic disease-modifying anti-rheumatic drugs. Whilst the advent of advanced therapeutics has improved clinical outcomes, a considerable number of patients remain unresponsive to treatment, emphasising the need for further understanding of disease progression and remission to support stratification of patients to treatment pathways.
